How to sign up

In order for NMIA to create an account for you, we require a signed original copy of our agreement with at least one month payment by cash, check, money order, or credit card.

Alternately you can fill out our online form.

New accounts are typically created on the same working day we receive your completed agreement with payment.

If you prefer to return the completed agreement by fax, please be aware that we must have a copy mailed to us with your original signature, and that payment must be guaranteed with a credit card.

To receive an application form by mail, leave your name and address at (505)247-0888 or send email with your name and address to new-accounts@nmia.com or send US mail to our mailing address:

If you call our voice line we can tell you where you can pick up the package, and eliminate the USMail delay.
